Customer-fault miles penalty when a customer cancels late: a no-show penalty if the driver had already arrived, otherwise a late-cancel penalty when the driver was en route and the trip was not already materially late through system/driver fault. Keyed on the cancellation actor (operator-asserted, since there is no CUSTOMER auth type) and the lateness attribution.
